Before scheduling
Do you want to review prevention, discuss a new symptom, or follow up on treatment? Explain your needs when you call. This helps arrange the appropriate visit and lets you ask about its cost before arriving.
What this benefit covers
Medicare’s annual wellness visit helps organize a prevention plan; it is not a routine physical exam. It is generally available once every 12 months after you meet Part B eligibility requirements. If the provider accepts assignment, this benefit has no charge; additional services may have costs.
What to bring to SOAR
Bring your identification, insurance card, medication list, and recent visit documents you want to share. Write down your three main questions. If someone accompanies you, tell them what you want to share and what you prefer to discuss privately.
If you have Medicare Advantage
Keep your full plan name and current card available. Ask about the provider’s participation, the visit type, and any additional services. One insurer may offer different products, so your specific plan should be checked before assuming coverage.
Make the visit answer your questions
You can ask: What should I address first? Are any documents missing? Who coordinates my next appointment? Before leaving, ask for the next step in plain language. If something is unclear, ask again.
Frequently asked questions
Is it the same as a physical exam?
No. It is a visit focused on a prevention plan.
Are all lab tests included?
Do not assume they are. Check the service and coverage before having it done.
